Ingredients:

  • 2 lb boneless, skinless chicken breasts
  • 8 oz diced deli ham
  • 6 oz shredded Swiss cheese
  • 1 (10.5 oz) can condensed cream of chicken soup
  • 4 oz full-fat cream cheese, cubed
  • 1/2 cup heavy whipping cream
  • 1 tsp garlic powder
  • 1/2 tsp black pepper

Instructions:

  1. Prepare the sauce base: In a medium bowl, whisk together the condensed cream of chicken soup, cubed cream cheese, heavy whipping cream, garlic powder, and black pepper until mostly smooth.
  2. Layer the chicken: Place the chicken breasts evenly across the bottom of the slow cooker insert. Pour half of the prepared sauce mixture over the chicken.
  3. Add ham and cheese: Sprinkle the diced deli ham and half of the shredded Swiss cheese over the chicken and sauce. Pour the remaining sauce mixture over the top.
  4. Cook: Cover the slow cooker and cook on the LOW setting for 4 hours, or until the chicken reaches an internal temperature of 165°F (74°C) and is fork-tender.
  5. Finish and serve: If desired, sprinkle the remaining Swiss cheese over the top during the last 15 minutes of cooking. Serve immediately.
